I recently had one rejected initially since Apple thought that the reason for the login process was to perform in-app purchases bypassing Apple. Took a few rounds of explanations and appeals to sort it out. You might want to state the reason for the login process in the comments when you submit the app.

When you submit the app from iTunesConnect, you will see the text fields for username and password in your App Metadata section. Just create a user and provide the details in the metadata. That should work. 
I've got my 5 apps approved which required login to start using the app.
